Heparin-induced thrombocytopenia associated with low-molecular-weight heparin: clinical feature analysis of cases and pharmacovigilance assessment of the FAERS database

Background: Unfractionated heparin (UFH) and low-molecular-weight (LMWH) are commonly used anticoagulants for the management of arterial venous thromboses. However, it is crucial to be aware that LMWH can, in rare cases, lead a dangerous complication known as heparin-induced thrombocytopenia (HIT). The objective this study was evaluate pharmacovigilance clinical features HIT associated with LMWH, well identify treatment strategies risk factors facilitate prompt management. Methods: We extracted adverse event report data from FDA Adverse Event Reporting System (FAERS) database assessment. Case reports on LMWH-induced dated up 20 March 2023 were collected retrospective analysis. Results: Significantly elevated reporting rates shown (AE) LMWHs FAERS database, while tinzaparin had higher proportional ratio (PRR) odds (ROR) than other LMWHs, indicating greater likelihood HIT. analysis indicated total 43 patients showed evidence median onset time 8 days. Almost half events caused by enoxaparin. mainly prescribed embolism thromboprophylaxis joint operation. Patients history diabetes or surgery appeared more susceptible Clinical symptoms mostly presented thrombus, skin lesion, dyspnea. 90% experienced platelet reduction 50% Warkentin 4T score 6, high In all patients, determined cause promptly withdrawn. Following discontinuation almost given alternative eventually achieved recovery. Conclusion: but serious, increased surgical history. Prompt recognition safe use LMWHs.
